Shoulder Pain: Causes, Symptoms and Treatment

The rotator cuff is a group of four muscles and tendons that stabilize the shoulder joint and allow the arm to move smoothly. These structures help control lifting, reaching, and overhead movement.

Rotator cuff injuries are a common cause of shoulder pain and can occur from repetitive strain, sports activity, lifting, or age-related tendon changes. Some people develop a rotator cuff strain or tendon irritation, while others may experience a partial or full rotator cuff tear.

Physiotherapy treatment focuses on reducing shoulder irritation, improving mobility, and strengthening the muscles that support the shoulder joint.

At Richmond Physiotherapist, we provide physiotherapy treatment for rotator cuff injuries in Richmond to help patients recover shoulder function and return to normal activities.

What Causes Shoulder Pain

Rotator cuff injuries often develop gradually due to repetitive movement or overuse of the shoulder.

Common causes include:

  • repetitive overhead activities
  • lifting heavy objects
  • sports such as tennis, swimming, or volleyball
  • sudden shoulder strain or trauma
  • age-related tendon degeneration
  • shoulder impingement

Rotator cuff problems may start as mild irritation but can worsen over time if the shoulder continues to be overloaded.

Symptoms of Shoulder Pain

Rotator cuff injuries can cause a variety of shoulder symptoms depending on the severity of the condition.

Common symptoms include:

  • shoulder pain when lifting the arm
  • weakness during overhead movement
  • pain when reaching behind the back
  • discomfort when sleeping on the affected shoulder
  • clicking or catching sensations in the joint
  • difficulty lifting objects

If these symptoms persist, physiotherapy assessment can help determine the underlying cause and guide treatment. pain symptoms can vary depending on the tissues involved.

How Physiotherapy Helps Shoulder Pain

Physiotherapy treatment focuses on restoring shoulder mechanics and reducing stress on the rotator cuff tendons.

During your physiotherapy assessment at Richmond Physiotherapist, we evaluate:

  • rotator cuff muscle strength
  • shoulder joint mobility
  • scapular (shoulder blade) movement
  • posture and shoulder mechanics
  • contributing muscle imbalances

Understanding these factors helps determine the most effective treatment approach.

Physiotherapy Treatment for Shoulder Pain

Treatment may include a combination of physiotherapy techniques depending on the severity of the injury.

Common treatments include:

Manual Therapy

Hands-on techniques used to improve shoulder joint mobility and reduce stiffness.

Exercise Rehabilitation

Strengthening exercises designed to improve rotator cuff function and shoulder stability.

Dry Needling / IMS

Helps release muscle tension around the shoulder and upper back.

Shockwave Therapy

May help treat chronic tendon irritation associated with rotator cuff tendinopathy.

Movement Education

Advice on lifting technique, posture, and activity modification to reduce strain on the shoulder.

These treatments help restore shoulder strength and reduce pain.

When to See a Physiotherapist

You should consider physiotherapy if:

  • shoulder pain persists for several weeks
  • lifting the arm becomes difficult or painful
  • shoulder weakness develops
  • pain interferes with sports or exercise
  • symptoms affect sleep or daily activities

Early physiotherapy treatment can often help prevent the condition from worsening.

Frequently Asked Questions

Do I need surgery for a rotator cuff tear?

Not always. Many partial tears respond well to physio, especially if you can still lift the arm against some resistance. Full tears where you physically cannot lift the arm are the surgical conversation. Most of the cuff patients I see never need the OR. We strengthen what’s left and get the shoulder working again.

What does a rotator cuff tear feel like?

Pain at the top and outside of the shoulder, worse with overhead reaching and lifting. Night pain when you lie on that side is classic. Weakness doing things like lifting a bag into an overhead bin. Sudden tears after a fall feel sharp and immediate. Gradual wear-and-tear tears creep up over months.

Can I still work out with a rotator cuff injury?

Yes, but modified. Cut overhead pressing, dips, and anything that reproduces sharp pain. Keep your lower body and core work going. We’ll rebuild the shoulder with targeted cuff and scapular strengthening, then progress you back to full training. Training around it beats stopping everything.

How long does rotator cuff rehab take?

Partial tears typically need eight to twelve weeks of progressive strengthening. Post-surgical rehab runs longer, usually four to six months before full return to sport. The cuff heals slowly because tendons have limited blood supply. Rushing it just resets the clock, so we build load gradually.
